Athletic Eligibility Requirements

All athletic physical packets must be completed and uploaded to the Aktivate web portal. Hard copies will no longer be accepted.  Parents and students can fill out the student and parent information on the online forms or print them out and complete them and then upload them onto the Aktivate website. The actual athletic physical paperwork must be printed, completed, and signed and stamped by the doctor and then uploaded onto the website. All athletes must have a sports physical completed. A birth certificate, for the student, is also required to be uploaded. Two of the papers MUST also be completed and signed in front of a public notary and then uploaded. Also, please make sure to fill out the insurance information for the student.

All paperwork must be completed and approved PRIOR to tryouts starting. When registering, please indicate all sports the student will be playing or trying out for. This way, you will only have to go through this process once a year. You will not have to do it for each sport. Please do not wait until the last minute and get the paperwork completed before the tryouts begin.
